If you pass the Charisma Check in Baldur's Gate 3 and can persuade him, Malus Thorm will agree, kill the patient, lie on the operating table himself, and tell the nurses to practice on him. They will then stab Malus and kill him.

To speak with him, you'll have to wait for an opportunity where the two of you are alone. You'll have an additional option if you successfully complete the persuasion checks on the rooftop. If you successfully pass the latest persuasion check, you won't have to fight against Thorm.
